We make choices based on our inner stories, our self-image, our self-compassion or self-shaming:

I need to hide. I don’t like my body. I mustn’t feel pleasure.
Life is hard and I have to face the tough world.
These ideas become movement behaviours, ways that we present ourselves. We direct or block our energy based on those stories which creates a context within which illness might tell our story if we don’t choose to listen before that becomes true.
